The risk of adverse drug reactions (ADRs) due to multiple drug use and aging-related pharmacodynamic or pharmacokinetic changes is higher in elderly patients than in other age groups. ADRs manifest as atypical symptoms and signs and are commonly confused with other diseases in elderly patients. This study analyzed the patterns of ADRs in the elderly patients using the data reported to the regional pharmacovigilance center of a single tertiary hospital.
This study retrospectively analyzed data on ADRs in patients aged 65 years or older reported to the regional pharmacovigilance center of Chosun University Hospital from January 2021 to March 2021. Variables analyzed demographic characteristics of the study subjects, drugs associated with the reported ADRs, symptoms, time of symptom onset, and drug causality.
The data included a total of 456 patients (57% female and 43% male). The mean age of the elderly patients was 72.9±5.9 years. Anti-tumor drugs (290cases, 63.6%) and narcotics (82cases, 18%) were the most common drugs to be associated with ADRs in the elderly patients. Nausea and vomiting (168cases) were the most common clinical manifestations of ADRs, followed by appetite loss (120cases), systemic weakness (39 cases), and leukopenia (36 cases). Of all patients, 79.6% experienced ADRs within 24 hours after drug administration. In 36.8% of the patients, the causal relationship between reported drugs and ADRs was deemed “possible”.
In this study, clinical symptoms (nausea, vomiting, systemic weakness, and appetite loss) due to anti-tumor agents and narcotics were frequently reported in elderly patients. The data used in this study may be used in anti-cancer medication counseling for elderly patients who are on anti-tumor agents. In this study, reports of ADRs were focused on anti-tumor agents and narcotics. To obtain more data on ADRs in elderly patients, pharmacists shall monitor ADRs in various medication groups during active medication counseling.
